Allergy Action Plan

This service is to request an updated ASCIA Action plan for your child.

Please request either a Red ASCIA (Anaphylaxis) or Green ASCIA (Allergy) Action Plan when prompted.
The plan you request must match the one that your child has previously been provided.
Please ensure that the ‘Review by’ date on your current plan has passed before ordering new plans.
Updated ASCIA Action Plans will ONLY be issued where; the current plan has passed the review date shown on the plan, your child is an existing patient of The Children’s Clinic and they have a future appointment booked for a review with their treating specialist.
Please ensure the above criteria are met as refunds can not be provided for incorrect ordering.
Your order will be processed as soon as possible. Please note that the specialists at The Children’s Clinic work on particular days of the week and therefore there may be a delay in processing your order. We apologise for any inconvenience.

Please confirm that your child is an existing patient at The Children's Clinic?
