Allergy Action Plan

This service is to request an updated ASCIA Action plan for your child.

• Please request either a Red ASCIA (Anaphylaxis) or Green ASCIA (Allergy) Action Plan when prompted.
• The plan you request must match the one that your child has previously been provided.
• Please ensure that the ‘Review by’ date on your current plan has passed before ordering new plans.
• Updated ASCIA Action Plans will ONLY be issued where; the current plan has passed the review date shown on the plan, your child is an existing patient of The Children’s Clinic and they have a future appointment booked for a review with their treating specialist.
• Please ensure the above criteria are met as refunds can not be provided for incorrect ordering.
• Your order will be processed as soon as possible. Please note that the specialists at The Children’s Clinic work on particular days of the week and therefore there may be a delay in processing your order. We apologise for any inconvenience.
